Implanted-pacemaker-induced dysrhythmia and its management.

چکیده
Case reports Case 1 A 65-year-old diabetic lady had been treated with Saventrine since 1965 for her heart block. In September 1971 she developed frequent StokesAdams attacks for which a ventricular inhibited intracardiac pacemaker was implanted. Within 24 hr of implantation she developed frequent ventricular extra-systoles which responded to procainamide. Subsequently she developed lignocaine-resistant ventricular tachycardia (Fig. 1) which responded to 20 mg of practolol, given intravenously. As the ventricular extra-systoles reappeared she was treated with practolol 100 mg twice daily and she has had no dysrhythmia since.

Pacemaker Twiddler's Syndrome
A 76-yr-old man with unipolar ventricular pacemaker, previously implanted in the right ventricular apex, presented with syncopal attack and twitching of the right pectoralis muscle. He finally was diagnosed as having Twiddler's syndrome. The patient had a pulse rate of 36/min and cannon waves in the neck veins. Chest radiography showed a dislodged pacing lead completely wrapped around the pulse...
متن کاملCardiac rhythm management devices (part I): indications, device selection, and function.
PACEMAKER1 and internal cardioverter–defibrillator (ICD) devices have undergone remarkable evolution since the first implantation of an asynchronous singlechamber pacemaker in 1958 and of an ICD in 1980. Today, more than 500,000 patients in the United States have pacemakers, and up to 115,000 new devices are implanted each year. The number of ICDs implanted each year has steadily increased, rea...
متن کاملA clinical appraisal
A technique for determining the characteristics of impulses from implanted cardiac pacemakers with external methods is reviewed, and its value in detecting pacemaker failures is discussed. The resultsfrom a 2-year application with 6ii controls in clinical routine are presented. Sixteen out of b*42 defective pacemakers could be detected before symptoms or electrocardiographic signs of pacing fai...
متن کاملDoes the stomach remain silent after neonatal loss of its original pacemaker?: gastric motility in long-term survivors of neonatal gastric rupture.
Gastric peristaltic contractions are controlled by an intrinsic electrical pacemaker located in the mid-body along the greater curve. This study was undertaken to investigate gastric motility in long-term survivors of neonatal gastric rupture who were surgically deprived of their original pacemaker. Four patients, 1 boy and 3 girls, aged between 6 and 12 years were studied. Physiological activi...
متن کاملSubclavian crush syndrome: a cause of pacemaker lead fracture.
A 48-year-old male with a symptomatic 2:1 atrio-ventricular block and a dual-chamber pacemaker, implanted one year previously, was admitted due to a syncopal episode. Pacemaker malfunction was identified as the cause of syncope. Subclavian crush syndrome was the cause of the pacemaker malfunction. Its incidence, consequences and management are discussed in this report.
